About the Role:
12 weeks contracts starting ASAP, working in the Community Team inQueanbeyan, NSW! Case management experience required.
Travel & Accommodation Covered + Daily Meal Allowance + Education Allowance
Responsibilities:
- Assessing and planning care for clients.
- Providing crisis intervention and medication management.
- Delivering therapeutic interventions and psychoeducation.
- Coordinating integrated care and support services.
- Conducting health promotion and education activities.
- Collaborating with multidisciplinary teams.
- Engaging with the community resources and stakeholders.
- Maintaining accurate documentation.
Requirements:
- Current AHPRA registration.
- A minimum of 2 years' mental health nursing experience, including adult community and casemanagementexperience.
