About the role: As a Community Mental Health Nurse, you will play a vital role in supporting individuals with mental health conditions in a community setting. You will be responsible for providing comprehensive case management, conducting mental health assessments, developing care plans, and working collaboratively with multidisciplinary teams to enhance the wellbeing of clients.
Key Responsibilities:
- Conduct mental health assessments and develop individualized care plans
- Provide case management and coordinate care for clients in the community
- Offer therapeutic support and interventions, including crisis management
- Collaborate with healthcare professionals, support services, and family members
- Monitor and evaluate the progress of clients, adjusting care plans as needed
- Promote mental health education and advocacy within the community
Requirements:
- Current AHPRA registration.
- Australian working rights (visa sponsorship is not available).
- A minimum of 5 years' recent experience in mental health nursing, including experience in community case management.
- Experience of working in the Australian healthcare system is strongly preferred.
- Post-graduate qualifications are highly desirable and may equate to additional pay.
