Role & Responsibilities:

Chronic Disease Nurse:

  1. Patient Assessment: Conduct comprehensive assessments of patients with chronic illnesses to identify their healthcare needs, challenges, and goals.

  2. Care Planning: Develop individualized care plans in collaboration with patients, families, and other healthcare professionals to manage chronic conditions effectively.

  3. Education and Counseling: Provide patients and their families with education about their chronic illness, including treatment options, medication management, lifestyle modifications, and self-care strategies.

  4. Monitoring and Evaluation: Monitor patients' health status, symptom progression, and response to treatment over time. Evaluate the effectiveness of interventions and adjust care plans as necessary.

  5. Medication Management: Administer medications, monitor medication adherence, and educate patients about potential side effects and interactions.

  6. Health Promotion: Promote healthy behaviors and lifestyle modifications to prevent complications and improve overall well-being. Offer guidance on diet, exercise, stress management, and smoking cessation.

  7. Collaboration: Collaborate with interdisciplinary healthcare teams, including physicians, pharmacists, dietitians, and social workers, to coordinate comprehensive care for patients with chronic diseases.

  8. Patient Advocacy: Advocate for patients' needs, preferences, and rights within the healthcare system. Help patients navigate complex healthcare systems and access resources and support services.

Sexual Health Nurse:

  1. Sexual Health Education: Provide patients with accurate and up-to-date information about sexual health, including contraception, STI prevention, sexual anatomy and function, and healthy relationships.

  2. STI Testing and Treatment: Conduct confidential testing for sexually transmitted infections (STIs) and provide appropriate treatment and counseling to patients diagnosed with STIs.

  3. Contraception Counseling: Offer guidance and support to individuals and couples in choosing contraceptive methods that align with their reproductive goals, preferences, and medical history.

  4. Pregnancy Counseling: Provide counseling and support to individuals facing unplanned pregnancy, including discussing options such as parenting, adoption, and abortion.

  5. Sexual Dysfunction Assessment and Support: Assess and address sexual dysfunction issues, such as erectile dysfunction, premature ejaculation, and low libido, through counseling, referrals, and treatment options.

  6. Risk Reduction Strategies: Educate patients about safer sex practices, including the use of condoms and other barrier methods, to prevent STIs and unintended pregnancies.

  7. Cultural Competence: Demonstrate cultural sensitivity and competence when discussing sexual health topics, respecting diverse beliefs, values, and practices related to sexuality and relationships.

  8. Confidentiality: Maintain strict confidentiality and privacy standards to ensure patients feel comfortable discussing sensitive sexual health issues without fear of judgment or disclosure.

  9. Cervical screening and sexual health checks

About the contract:

  • Up to $108.42
  • Dates: May 01st- 9 weeks 
  • Accommodation and travel provided – flights booked and paid for, or the cost of petrol reimbursed
  • $500p.a. training and education allowance (applicable after 12 weeks worth of work)
  • Must have ALS, immunisation, STI & Syphilis certificates (free to do) Pharmaco
